To register by phone, please call 877.ICU.CLASS (877.428.2527) and
have the following information ready:
- Name, date, and location of the course for which you'd like to register
- The number of people you are registering and their full names
- Your name
- Your occupation
- Home address
- Home phone number
- Work address
- Work phone number
- Credit card information--card number, expiration, name as it appears
on the card (please clarify if your billing address is different from
your home address)
